Drivers will also often participate in team meetings, providing valuable information to the PACE center manager, and primary care staff about how the patient transfers and the home environment. Our drivers are responsible for other tasks outside of driving as well. If you’re looking for a position that is slightly different each day. There is no one day like another with this position.

· NO CDL required!

· Experience driving larger vehicles is preferred.

Program Objectives:

The Michigan Coalition Against Homelessness AmeriCorps Program will support service providers by increasing their capacity to offer assistance and services to homeless and at-risk individuals and families. As an overall program, AmeriCorps members in the program provide direct, comprehensive services to those experiencing or at-risk of homelessness in order to bring those individuals under the scope of services, assist with increasing self-sufficiency in the areas of income, life skills, and employment, and assist with obtaining/maintaining employment. Members will fulfill their specific positions, based on the needs of their host site organization, to contribute toward this increased capacity to serve their communities more efficiently and effectively.

Classification:

According to the National and Community Service Act of 1990, AmeriCorps members are not considered employees of the agency where they serve, nor are they employees of the program or the federal government. As such, members cannot engage in activities performed by employees of the host agency or otherwise displace employees.

Essential Duties and Responsibilities:

● Assist with intakes to assess the housing needs of that client

● Address housing barriers for the client

● Create/update lists of available housing for the clients to choose from

● Connect clients with necessary funds to pay for security deposits, rental fees, or utility payments

● Maintain/develop relationships with local landlords

● Receive training for/conduct Housing Quality Standards (HQS) and other housing inspections

● Assist with independent living skills support related to addressing income and housing stabilization needs

● Gather and disperse food and clothing donations to clients

● Facilitate delivery of needed household items to clients who receive housing

● Generate information materials for clients, such as newsletters/pamphlets/etc.

● Provide follow-up support services as needed

Training:

Members are expected to participate in program and host site training throughout the service year, as scheduled and requested by the program. Basic training events will include member orientation, mid-year training, and end-year training. The program will also schedule various member conference calls and webinar topics. Host sites will be responsible for conducting an initial on-site orientation and training, as well as providing necessary continued training related to the service activities and professional development. For program-held training events, members will be provided the date and details at least three weeks in advance.

Service members will receive the following training during their service

● Homeless Management Information Service database systems entry

● Introduction to grant writing

● Introduction to strength-based interviewing

● Time Management

● Professional Development Planning

Restrictions:

No more than 20 percent of the member’s total time can be spent on fundraising or training activities.

● Absolute ability to maintain consumer and agency confidentiality;

● Ability and willingness to learn to build rapport and maintain professionalism with clients;

● Ability and willingness to learn to develop skills to provide assessment, case management, and follow up services to clients;

DIRECT CARE WORKER

General Function

The Classroom Paraprofessional shall carry out those duties and activities assigned which will ensure the effective operations of the class or activity to which they are assigned.

SUMMARY

Reporting to the Hunger Prevention and Children’s Services Director, this multi-function position will

a. Assist in coordinating TrueNorth Hunger Prevention programs and volunteers to ensure successful service delivery

b. Help Manage Hunger Prevention program inventory including storing and stocking

c. Assist with the coordination, preparation, and execution of Children’s Services including Tools for School, Car Seats, Coats for Kids (& Adults), and Children’s Christmas Programs.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

● Promote wellness through nutrition and education;

● Assist in managing the program database and work with the Director to create and share program utilization data regularly with agency staff;

● Maintain food storage and program areas;

● Receive & physically unload deliveries; lifting, moving, unpacking, and organizing boxes and supplies up to 70 lbs.;

● Help document and report all food inventory activities including donation intake and orders through preferred vendors;

● Ensure that confidentiality, dignity, and non-judgmental service to participants is maintained at all times by staff and volunteers;

● Help Administer DoubleUp Food Bucks program;

● Work with the Director to develop and maintain Children’s Services calendar and timeline; provide updates to TrueNorth staff and community partners;

● Share and process Children’s Services program applications;

● Coordinate in-kind program donations;

●Help Recruit, train, place, supervise, and recognize Hunger Prevention and Children’s Services volunteers;

● Regularly operate organization vehicles, pallet jacks, and hi-lo;

● Help complete Food Rescue pick-ups in the community throughout the week in organization vehicle;

●Adhere to best practices in volunteer management and strive to provide volunteers with meaningful and satisfactory experiences;

●Conduct surveys, maintain data, and solicit feedback from program participants and volunteers to evaluate satisfaction and identify unmet needs;

● Assist with Weekend PowerPack assembly & delivery;

● Collaborate with Volunteer Services to:

o Share volunteer data and feedback;

o Stay abreast of industry trends and complete professional development in volunteer management.

SUPERVISORY RESPONSIBILITIES: This position has volunteer supervisory responsibilities. The Hunger Prevention & Children’s Services Specialist will carry out supervisory responsibilities in accordance with TrueNorth policies and applicable laws. Responsibilities include intake, training, placement, and management of volunteers; recognizing volunteers; addressing complaints and resolving problems.

EDUCATION and/or EXPERIENCE: High School diploma or equivalent, experience working with volunteers and vulnerable populations. Associates or Bachelor degree in related field preferred. Experience working with food preferred.

Responsibilities may include:


Assist the facility with:

Meeting the resident/guest on or shortly after the admission and following them until they discharge to the community.

Attend the 72 hour care plan meeting

Ordering equipment for the community setting

Arranging home health nursing for after discharge, as needed

Coordinating the medications for community use

Coordinate the entire discharge, with IDT involvement

Attending care conferences for sub-acute residents/guests

Attending Medicare Meetings

Assist in procuring any item related to the discharge process for residents/guests anticipated to be returning to the community.

Initiate and coordinate completion of the Post Discharge Plan of Care

Develop an individualized discharge care plan for the resident/guest discharge in collaboration with the facility, the family, and the practitioner, assuring the resident/guest receives the care that will meet their needs in the community.
